Trademark application requirements
The following is a list of the required fields needed when completing your trademark application.
- Your name.
- Address, city, state, country and zip code.
- Telephone number.
- Email address (recommended for ease of contact if examining attorney should need to contact you).
- Country of citizenship.
- What type of entity your business is.
- The state where you conduct business.
- Date of first use.
- Your class number for the products or services you offer. (Each class is $325 US Government Fee).
- If you are not already using the trademark you will need to provide proof that you intend to use the trademark, and the date of intended use.
- If you have a logo you are registering in the trademark application you will need to email the logo to info@trademarks411.com.
