Trademark application requirements

The following is a list of the required fields needed when completing your trademark application.

  • Your name.
  • Address, city, state, country and zip code.
  • Telephone number.
  • Email address (recommended for ease of contact if examining attorney should need to contact you).
  • Country of citizenship.
  • What type of entity your business is.
  • The state where you conduct business.
  • Date of first use.
  • Your class number for the products or services you offer. (Each class is $325 US Government Fee).
  • If you are not already using the trademark you will need to provide proof that you intend to use the trademark, and the date of intended use.
  • If you have a logo you are registering in the trademark application you will need to email the logo to info@trademarks411.com.
